Community Support Services After Discharge

When a student is placed in Residential Services, we believe planning for discharge must begin at the point of admission. Towards that objective, LaSalle created ‘Community Connections’ to provide family based counseling and support services concurrently with campus based residential services. The Community Connections Worker focuses on the barriers in the home and community which put the student’s ability to achieve permanency within his family at risk. This worker coordinates the family services necessary to support the youth’s treatment plan and his eventual transition back home. The worker supports the family’s participation in treatment planning and assists to assure the family is well engaged in services both at LaSalle and in their own community. As the student returns home, the worker stays involved to guide that important transition and connect the family to achieve and sustain stability. We partner with each County to identify the role Community Connections might serve with families they refer to our Residential Services Program.
