    Our highly credentialed team of clinicians provide state of the art, trauma informed, evidence based treatment services for youth and families along a continuum of care.     Case Manager/Social Work Associate

  • LaSalle School is seeking to hire a full time, experienced, and dedicated Case Manager to join a skilled and diverse team of professionals who serve the needs of youth and families enrolled in residential foster care and mental health programming. The Case Manager will provide counseling/case management services to at-risk adolescent males and their families.

    Our programs are licensed by NYS OCFS, OASAS and OMH and are fully accredited by the Council on Accreditation, a highly regarded independent national accrediting body for organizations in child welfare.

    Mandatory Qualifications:
     Valid NYS Driver’s License
     Bachelor’s Degree in Social Work, Psychology, or other relevant field
     Demonstrates motivation to positively support agency mission, philosophy and expectations
    Desired Qualifications:
     Two, or more, years of experience in providing case management support to youth/ families engaged in foster care, family court, juvenile justice, mental health programming.
     Familiar with basic principles of relevant local services and systems supportive of adolescents with behavioral and emotional problems.
    Overview of Position
     Provides case managements supports and services to assigned youth within foster care/ juvenile justice setting.
     Partners with assigned clinical and agency staff to advance treatment and permanency goals.
     Assists with all relevant OCFS, court and other relevant documentation.
     Acts as treatment team and agency liaison to family and community partners (e.g., family court, DSS, Probation, etc.)
    General Responsibilities/Duties
     Works closely with intake and clinic staff to assist with case related duties
     Assists with documentation and case record maintenance in all relevant systems (CONNECTIONS, LSS CIS, IEP Direct etc.)
     Communicates critical information in a timely manner to all relevant partners and stakeholders.
     Provides transportation and assistance to youth and families (e.g., family court, medical appointments, home visits, probation, etc), as assigned.

    Necessary Job Skills:
    Excellent verbal and written communications skills; Ability to follow through on case activities and assignments in a prompt and professional manner; Excellent interpersonal skills, family engagement skills; Ability to work well with a diverse treatment team; Ability to enter data and maintain electronic records in a timely manner.

  • Care Manager

  • LaSalle’s School is seeking to hire a full time Care Manager to join our Clinical team of professionals serving the needs of our youth across residential and outpatient programming. Our programs are fully accredited by the Council on Accreditation, a highly regarded independent national accrediting body for organizations in child welfare. Our most recent review in stated that our entire organization “is a model and a leader for other like agencies in the state and nation.” The Therapist will provide comprehensive clinical and case management services to assigned adolescents and families in any of LaSalle School’s program areas.

    Minimum Qualifications:
     Bachelor’s degree with two (2) years relevant experience or Registered Nurse with two (2) years of relevant experience or Master’s degree with one (1) year relevant experience.
     Valid New York State Driver’s License
     Relevant experience in serving children and families in child welfare, developmental disabilities, behavioral health, primary health care, or social services.
     Experience coordinating and participating in team settings.
     Experience with a range of special populations, to include experience with children/youth with substance abuse and/or complex medical conditions.

    Position Responsibilities:
     Document all case activity, including outreach, consent development and assessment, plan development, client progress and transition arrangements.
     Develop a comprehensive, family-driven youth-guided Plan of Health Care.
     Inventory and coordinate existing services relevant to the Plan of Care, identifying and securing additional services as appropriate.
     Provide education to family and child concerning the nature and desired effect of services, along with information concerning conditions being treated.
     Assist families and children in the acquisition and maintenance of public benefits e.g., financial, educational, social, and community services.
     Assure responsible transition of client service into and out of Health Home care, between child and adult health homes, and between inpatient and community care as appropriate.

    The purpose of the Juvenile Reporting and Family (JRFC) is to introduce at-risk adolescents to skills, activities, and opportunities; tap into their strengths and foster appropriate peer and adult relationships; and help them become successful members of their community. The LaSalle site serves males ages 13-16. This Alternative to Detention and Prevention program provides for Albany County youth known to probation, who live at home and attend local schools, a supervised, structured array of enrichment recreational activities during high-risk evening hours. The program is built upon the strategies of Positive Youth Development.

  • Residential Living Staff

    Responsibilities include providing individual, family and group counseling services for residential students and their families; experience working with at-risk JD/PINS adolescents and their families, providing family counseling, support and linkages to natural and community based supports; experience working with a treatment and team based service system; knowledge of NYS OCFS Connections electronic record system.

    Youth Development Specialists [Direct Care]

  • Responsibilities include care and supervision of students in their residential setting. The Child Care Counselor is responsible for providing a safe, therapeutic residential experience for all children.

    Available Shifts:
    Full Time Days includes evening and weekend hours

    Overview of Position:
    The Night Child Care Worker is responsible for the active and effective supervision and care of residents in their assigned division. Awake and alert, situated where the movement and activities of residents can be closely observed, providing for the safety of everyone. The Child Care Worker works overnight hours and during early morning hours, assuring the routine and procedures of the division are followed. The Child Care Worker monitors the safety and well being of all residents during sleeping hours and directs them through their morning routine. Worker must be familiar with the ICMP, treatment issues, and safety plans of residents in their division. Must effectively implement all aspects of active supervision, and established behavior management strategies. Must be familiar with the use of all technology which allows for the security of the environment and the exchange of information with supervisors and colleagues.
