Health Insurance

CDPHP (Capital District Physician’s Health Plan)

Eligibility

  • Available to all Regular Full-time employees [unless under the grandfather provision] (regular full-time constitutes assigned to work 35 or more hours per week)
  • Waiting Period 90 days

Cost

  • Individual coverage: LaSalle pays 100% of cost
  • Employee +1: LaSalle pays 50% of Cost.
    ($219.55 per pay period for 26 pays)
    ($271.82 per pay period for 21 pays)
  • Family coverage: LaSalle pays 50% of cost.
    ($308.99 per pay period for 26 pays)
    ($382.55 per pay period for 21 pays

Dental Insurance

(Assurant Employee Benefits)

Eligibility

  • Available to all Regular Full time employees. (35 +hours per week)
  • Waiting Period 6 months (1st of the month following 6 months of consecutive employment)

Cost

  • Individual coverage: LaSalle pays 100%  of cost
  • Family coverage: LaSalle pays 50% of Cost.
    ($19.41 per pay period for 26 pays)
    ($24.04 per pay period for 21 pays)

Life Insurance

(Mutual of Omaha)
LaSalle provides a group term life and accidental death & dismemberment policy of $50,000 per employee.

Eligibility

  • Available to all Regular Full time employees (35 + hours per week)
  • Waiting Period: 6 months (1st of the month following 6 months of consecutive employment)
  • Cost La Salle pays 100% of cost

Voluntary (Supplemental) Benefits

Applies to employees classified as Regular PT (20 or more hours per week) and FT (35 or more hours per week)

  • Vision Insurance: Eye Med (90 day waiting period)
  • Life Insurance Buy Up: Mutual of Omaha (same waiting period as the group policy)
  • Short Term DBL: Colonial (90 day waiting period)
  • Cancer Care Plan: Colonial (90 day waiting period)
  • Accident Care: Colonial (90 day waiting period)

Paid Leave

LaSalle provides paid time off through Vacation, Sick, Personal and Holiday pay. Specific eligibility is outlined in the personnel manual.

Pension Plan

Employees of LaSalle School who have 1 year of service and work at least 1,000 hours per year are eligible for the profit sharing plan. Employees are also welcome to set up a non-contributory 403 (b) plan from first day of employment.

Flexible Spending and Cafeteria Plans

LaSalle staff may elect to use pre-tax dollars to fund accounts used to pay for unreimbursed medical and dental expenses, dependent care expenses, and health and dental insurance premiums. (Waiting period and eligibility consistent with the medical insurance plan)

LaSalle will reimburse employees for up to 6 credits per fiscal year (7/1 – 6/30) for work related, preapproved courses, minus any Financial Aid received.
